Course Description:

This conference will allow social workers to enhance thr knowledge of the following: strategies to improve access to care for patients; preventive strategies, behavioral health integration; importance of pharamcy and oral health as part of the integrated team; treatment of subtance use in collaborative care settings using CoCM, addressing stigma and best practices for treatment of patients with HIV/STI; patients with diabetes and prediabetes; behavioral health screening and intervention in primary care; health literacy and improving equity for care; utilizing data in healthcare to improve patient resources and outcomes; collaboration with community health workers.
