Additional Breakout Forms

If you are a continuing education provider you are required by Michigan law to maintain records of course attendance that reflects the following information:

  1. The date and location of the program or course.

  2. The credentials of the course presenters.

  3. Rosters of the course attendees.  This must include a "sign in" and "sign out" time next to each attendees printed name and license number.

  4. The continuing education hours that were awarded to each attendee.

Guidelines require continuing education providers to maintain the following information for at least seven (7) years:

  • Names and resumes of CE directors and social work consultants

  • Participant name, profession, and license/certification/registration number from the appropriate jurisdiction

  • Course Title, Date, Location and CE hours awarded

  • Course outline/syllabus and learning objectives

  • Course instructor's qualifications and professional affiliations

  • ADA requests and services provided   (ADA - Americans with Disabilities Act)

  • Grievances and resolutions

  • Course evaluations


Provider Options

Individual Provider

Individual Provider pays for each CE Provider Application separately. Fees are determined based on two factors: (1) whether the organization is non-profit or for-profit and (2) how many days long the event is. Organizations should consider becoming an Individual Provider if they are only seeking CE credit approval for a single isolated event or if they are only seeking CE credit approval for 1-2 events per year.

Unlimited Provider

Unlimited Providers pay one annual fee and does not pay for subsequent CE Provider Applications (excepting applicable rush fees) for events that occur within 1 year of the initial event start date. Fees are determined based on whether the organization is a CSWE-accredited school of social work, non-profit, or for-profit. Organizations should consider becoming an Unlimited Provider if they are seeking CE credit approval for multiple events per year.

Rush Fees

Applications submitted 30-60 days prior to the event start date will be assessed an automatic rush fee for their application. Rush fees are in addition to the application fee(s) listed above. Applications assessed a rush fee will undergo an expedited review (1 week vs 3 weeks). As a note, applications submitted less than 30 days prior to the event start date will not be accepted.
