Course Description:

Creating behavior change in patients is a key component to better health outcomes. Social Workers are increasingly being called upon to provide behavioral interventions for multiple chronic health conditions. Additionally, the connection between various chronic health conditions is growing stronger. Providing integrated health care is quickly becoming the expectation, rather than the exception, in the health care setting. This course will review standard behavioral interventions for chronic health conditions, review initiatives that support the provision of integrated care, and explore the connection between chronic health conditions
