Improving Outcomes in Diabetes Mellitus. Modules A - E
Diabetes mellitus (DM) is a chronic metabolic disease that affects approximately 1 in 12 Americans, and which causes substantial illness and disability, resulting in high medical costs. This course, over five modules, will detail the prevalence of diabetes and its risk factors, as well as describe its diagnosis and assessment, and then its treatment and management. Next, it will explore the case manager’s role in diabetes care: that includes coordination of patient care in a variety of settings; contributing to the multidisciplinary diabetes care team; promoting patient self-management skills; advocating for the needs of diabetes patients; and supporting organizational goals related to diabetes care.
