Issues in Aging Day Two: New Enhancements in Frailty Care

Course Description:

Following this workshop, SW will be able to determine how to adequately evaluate older adults for evidence of frailty and sarcopenia and the appropriate interventions for management and prevention through addressing behavior modification.  Also, SWwill be able to identify trends in psychotropic prescribing among older adults, both of single agents and combination use, and understand the adverse impact of these drugs on cognition as we age.  SWwill be educated in the research trends and new treatment for Alzheimer’s disease and MCI which will lead to educated conversations with patients. Lastly, SW will understand the role that evidence -based model of care transitions from hospital admission to discharge back into the community and how it has been enhanced over time, and the value added for these teams as far as 30 re-admission data.
