Social Work Continuing Education Collaborative

These fees are valid for a period of 12 months.  Twelve months start on the date of your first presentation.

CSWE Unlimited          $500

 Non-Profit:

Unlimited -                   $750.00**

Individual event -       $150.00 (1 day event)

Individual event -       $200.00 (2 day event)*

 For Profit:

Unlimited -                  $1,500.00**

Individual event -         $300.00 (1 day event)

Individual event -         $400.00 (2 day event)*

* Events exceeding 2 days will require an additional $75.00 for each additional day.

 ** All Provider sponsored programs must be submitted for approval during the twelve month period.  No additional fee will be required for approval.

Preferred method of payment is check or money order.

 All applictions must include:

• Typed or handwritten (must be legible) application form.
• All required documents
• Payment in full

Make sure that all questions on the application are answered in full.  Applications with missing information will be placed on hold.
