Continuing Education application procedures and downloads. The Provider Application Process There are three types of Continuing Education Provider status:
CSWE accredited status is suitable to CSWE Schools of Social Work accredited by the Council on Social Work Education. -
This
provider can offer an unlimited number of postgraduate programs through
their continuing education program for a twelve month period. -
This approval expires twelve months from the original date of approval filed with the Collaborative. -
Individual programs must still be submitted for approval during this period, no additional fee is required. CSWE non-accredited status applies to the following sponsorship types: -
Unlimited
- Sponsor can provide unlimited number of programs in a 12 month period
coincident with the anniversary date of approval by the Collaborative. -
Individual Program - Provider can offer one program during a 12-month period. Provider approval is valid for one year. Non-Profit category
applies to any agency or organization that has a 501(c)3 status and is
not an accredited CSWE program. This sponsor can offer unlimited
or individual workshops. For-Profit
category applies to any agency, organization or corporation that does
not meet the criteria for a 501(c)3 status and is not an accredited
CSWE program. This sponsor can offer unlimited or individual
workshops. 1. When to apply -
Applications are processed on a first come first serve basis. -
Applications will be expedited if the Collaborative caused the delay. -
The Collaborative urges sponsors to apply at least 60 days before the first date of the program. This will allow enough time for review and process of your application. -
Once approved a provider number will be assigned and available for print on your program materials. 2. Processing the Application -
Once
the correct, thorough application and all required and supporting
documents are received, the Collaborative will review and process the
provider's request for approval.

Please allow two to four weeks for the review process. Again we urge you to submit application at least 60 days prior to the first program (workshop) date. 3. Canceling a Program If
you are an employer and wish to become a provider, download the
following forms from the document list at the bottom of this
page. Please make sure that you download all three of the forms
listed below. The Checklist document has important information
about ADA and Grievance requirements. The Sign In and Sign Out form is also a mandatory requirement for your application. You may submit this application and all required documents to the Collaborative digitally to: mingus@nasw-michigan.org.
