Interdisciplinary Collaboration for a Patient with Multisystem Impairments Following Hemorrhagic Stroke

Course Description:

Patients who have experienced a hemorrhagic stroke are often left with notable complex impairments that require effective collaboration of interdisciplinary teams. Methodical documentation of the patient’s rehabilitation journey and effective strategies employed by the interdisciplinary team are required to achieve optimal care delivery for these patients. This presentation will highlight a case study to provide an overview of the value of interdisciplinary care for individuals after a hemorrhagic stroke including recommendations for effective interdisciplinary team collaboration to enhance patient-centered care. Additionally, strategies to overcome barriers that impede interdisciplinary team work will also be presented.
